Longevity Health, San Francisco
Longevity Health is a concierge practice built around longevity first, with primary care included. It names the markers it runs: ApoB, lipoprotein(a), a coronary calcium score, DEXA, VO2 max and two weeks of continuous glucose. You are paired with a physician, dietitian, exercise physiologist and strength coach who meet weekly. Care happens in person and remotely.
No clinic to visit, so here is the actual path.
This is a national program rather than a practice with premises. Each step below happens somewhere different, and not all of them are run by the same company.

The care team
Every clinician the operator publishes by name, with their role exactly as the operator states it.
Longevity Health publishes one team across the whole practice: four physicians on its Physicians page, and a four-person care team of one physician, a dietitian, an exercise physiologist and a strength coach on its Care team page. The San Francisco page carries the heading "Four experts. One coordinated plan." and then prints that same four-person care team, identical to the one on the practice's home page, so nobody is published as the physician for San Francisco in particular. The practice publishes that each member is paired with their own physician, dietitian, exercise physiologist and strength coach from day one, without saying who. The Physicians page and the Care team page give Dr Valadao two different titles and name a different person as Clinical Lead, so neither carries a lead title.
