The treatments, in plain English

These treatments sit at the experimental edge of longevity care. A few have real but narrow medical uses, like hyperbaric oxygen for hard-to-heal wounds. For aging itself, the evidence is thin, and some carry real risks. Go in with clear questions, and be wary of big promises.

Think of these like new restaurants with no reviews yet. Some may turn out great, but right now you're mostly going on the chef's word.

Peptide therapy

Short chains of amino acids, often injected, sold for recovery, sleep or muscle. Most are not FDA-approved for these uses, and the FDA has flagged some as possible safety risks.

NAD+ therapy

An IV drip or shot of a molecule your cells use to make energy. Human studies so far are small and short, so the benefit for aging is unclear.

Cash price: $36 to $800

Hyperbaric oxygen

Breathing pure oxygen in a pressurized chamber. An established hospital treatment for things like carbon monoxide poisoning, the bends and hard-to-heal wounds. Anti-aging uses fall outside that list.

Therapeutic plasma exchange

Removes and replaces the liquid part of your blood. A standard hospital treatment for certain blood, nerve and immune diseases. Using it for aging is early research.

Ozone and blood filtration

Mixing ozone into your blood, or filtering it, for a "detox". The FDA describes ozone as a toxic gas with no known useful medical use.

Stem cells and exosomes

The only FDA-approved stem cell products are cord-blood cells for certain blood disorders, and no exosome products are approved. Treatments sold for aging, joints or the brain are unapproved.

PRP injections

Your own blood, spun to concentrate platelets, then injected into a joint, tendon or skin. Some support for tennis elbow and knee arthritis, little for most other uses.

How to approach these treatments

  1. Ask what it is for

The exact goal, and what result would count as it working.

  2. Ask for the evidence

Which studies support it, and in people like you.

  3. Ask about safety

Who makes the product, the side effects, and who handles problems.

  4. Track it

If you go ahead, set a date to judge whether it actually helped.

If you have a serious condition, don't swap proven treatment for any of these.

Common questions

Which clinics in Raleigh-Durham publish advanced and emerging care?

5 in our Raleigh-Durham guide: BioFunctional Med, D5 Health, Staywell Health, Triangle Wellness and Anti-Aging and The Hansell Center for Functional Medicine.

Are these treatments FDA-approved?

Mostly not for longevity. Hyperbaric oxygen and plasma exchange are standard hospital treatments for specific conditions. The FDA has approved no exosome products, and only cord-blood stem cells for certain blood disorders.

Are peptides safe?

It depends on the peptide and who makes it. Most aren't approved for the uses clinics sell them for, and the FDA lists some as possible safety risks in compounded drugs. Ask exactly which peptide, where it's made, and what side effects to watch for.

Does NAD+ therapy slow aging?

It hasn't been shown to. Your cells use NAD+, and it's an active research area, but human studies of boosting it are small and short so far.

What should I ask before trying one?

What it's for, what evidence supports it in people like you, who makes the product, what the side effects are, and how you'll know if it worked.

What does it cost, and will insurance cover it?

These are almost always self-pay. Insurance may cover hyperbaric oxygen or plasma exchange only for approved medical reasons. Our guides on cost and insurance go deeper.

How do you decide which clinics appear on this page?

A clinic appears here when its own published material shows it offers this kind of testing at that location.
